General Information
- Risk Assessment Process:
- Use your UBD email domain to send risk assessment documents to she.ihs@ubd.edu.bn for Faculty SHE Committee to review paperwork.
- If your risk assessment documents require amendments, the Faculty SHE Committee will provide advise/recommendations and reply to you by email.
Upon satisfactory submission/amendments as indicated by Faculty SHE Committee, (i) send risk assessment documents along with applicable checklist (see right column) in hardcopy to the "Risk Assessment Forms (NEW APPLICATION)" in-tray located at entrance of staff office on Level 2, Admin Block, IHS Extension Building, and (ii) notify Faculty SHE Committee by email (she.ihs@ubd.edu.bn) regarding the document shipment.
After Faculty SHE Committee verifies the risk assessment documents, you will be notified by email to collect your hardcopy from the "Safety Health and Environmental (VERIFIED)" out-tray located at entrance of staff office on Level 2, Admin Block, IHS Extension Building.
- Pass your risk assessment documents to Faculty Administrative Support Team for approval by Faculty Dean/Deputy Deans [and if applicable, for checking by Discovery Year Coordinator].
- Within two days of obtaining relevant signatures, submit scanned copy of verified and signed risk assessment documents to she.ihs@ubd.edu.bn for record and follow up. This is important especially in cases of incidents occurring.
- The risk assessment documents should be submitted to UBD Office of Safety, Health & Environment (UBD OSHE).
- You should retain a copy of the verified and signed risk assessment documents for your record.
For Downloading & Printing
- If your UBD-affiliated event is to be conducted outside of UBD Tungku Link campus premises, the risk assessment must include the Safety Management Plan.
Student events include, but are not limited to Discovery Year (DY), Final Year Project (FYP), Special Study Modules (SSM1, SSM2, SSM3), etc.
Staff events include, but are not limited to Human Research Development (HRD), research activities, etc.
- If there are multiple participants in the event, all participants must sign the risk assessment documents.

Faculty Risk Assessment Process Summary
- Staff conducts risk assessment and submits above document to Faculty SHE.
- Faculty SHE reviews and verifies risk assessment.
- Faculty Dean approves documents.

Faculty Risk Assessment Process Summary

- Staff forwards RPRA Section C only to IHSREC as evidence part of application for research ethics approval.

Faculty Risk Assessment Process Summary
- Student submits the above documents to Faculty SHE.
- Faculty SHE reviews documents, and verifies risk assessment.
- Student forwards (i) RPRA Section C and (ii) verification page to IHSREC as part of application for research ethics approval.
Instructions and Guidance for Students with UBD-Affiliated Activity at Clinical/Event/Fieldwork Sites (applicable to those undertaking SSM1, SSM2, SSM3, relevant FYP, and other relevant research-based modules)
- Sections A-C in RPRA Form
- Fill in as if you are the Principal Investigator.
- Section A in Student ABRA Form
- Fill in as accurately as you can.
- Interpret 'travel' / 'trip' / 'event' as 'module'.
- Section B in Student ABRA Form
- Identify relevant risks according to your module activity.
- You are expected to tailor Section B to meet the risk assessment requirements.
- Verification Page in Student ABRA Form
- This page is verified as long as the first 2 out of 4 tables are completed.
- Do not get the risk assessment checked by Faculty Discovery Year Coordinator.
- Next of Kin Details in ABRA Form
- Fill in.
- Safety Management Plan in ABRA Form
- This is mandatory.
- Indemnity Form and Parental Consent
- This is mandatory.
